Patients who complete all of their recommended treatment, always give 48 hours notice for a cancelled/rescheduled appointment, and keep all of their preventative hygiene appointments are eligible! Upon agreeing to the terms you will have your custom bleaching trays made for you at no charge and be given 3 tubes of our bleaching gel. Upon return to your hygiene appointments (every 3,4, or 6 months as determined by your treatment schedule) you will recieve 3 tubes of bleaching gel, and continue to recieve them as needed at each hygiene appointment! Patients are responsible for a $25.00 replacement fee for lost trays. Patients who fail to keep their hygiene appointments must pay a $25.00 fee to re-enter the program.
